Two years have passed since Jin Minakata (Takao Osawa) slipped through a time warp and landed in the end of Edo period. Worried about the state of depression into which Saki Tachibana (Haruka Ayase) has fallen, Jin pays a visit to her parent's home when it is revealed that her mother, Ei (Yumi Aso) has been stricken with beriberi disease. According to her elder brother Kyotaro (Keisuke Koide), Saki had come home to urge her mother to see a doctor only to be sent away without even being allowed in the home as Ei had not yet forgiven Saki for previously breaking off an arranged marriage proposal. In hopes of somehow saving Ei, Jin comes up with a scheme to administer medicine-laced sweets, for which Ei has a weakness.
Then one day, Ryoma Sakamoto (Masaaki Uchino) appears in search of Jin's help. It seems that the great military thinker and philosopher, Shozan Sakuma (Masachika Ichimura), beloved mentor to Kaishu Katsu (Fumiyo Kohinata), had been attacked by an unknown assailant and mortally wounded. Jin accompanies Ryoma to Kyoto, but worries that saving the life of such an important man runs the risk of greatly altering history. As expected, they find Shozan in such a critically ill state that it is a miracle he remains alive. Jin's desperate emergency treatment manages to save Shozan's life. But the first words uttered by the patient upon regaining consciousness provide the biggest surprise.
Jin remains in Kyoto, devoting himself to treating the injured from the heavy conflict between the Choshu and Satsuma clans. But he is soon apprehended by members of the shogun's guards, Shinsengumi, who show up suddenly and take him to Saigo Takamori (Takahiro Fujimoto) in the Satsuma clan headquarters; for Saigo is in need of immediate appendicitis surgery!
